Checking and savings account statements (past 2 months)
Evidence of other assets: bonds, stocks, retirement accounts (401k, 403b)

Income Verification

Recent paycheck stubs (most recent 30 days)
W-2 forms (past 2 years)
Federal tax returns (past 2 years)
If self-employed: profit/loss statements and business tax returns

Employment

Name and address of employer(s) for verification
